The following forms and information are needed for your child's camp session. Please print the Adobe Acrobat documents, fill-in, and bring to camp during Camper Check-in or mail to camp ahead of time.

Individual Forms
The following are links and discriptions of each individual form.
Medical Form
Wisconsin State Law requires us to have a health history on each camper.
This form can be mailed to camp or brought on the day of camper check-in.
This form must be completed and signed by a parent or legal guardian. In the case of emergency this form will allow Phantom Ranch to
authorize emergency medical treatment.
2010 Summer Camp Medical Form
Permission Form
Phantom Ranch requires that each camper has a signed permission form turned in for each camper. This allows attendance at camp, use of images for advertising (while adhering to
our privacy policy), and participation in activities both on and off camp grounds.
The form also has a Paintball Permission Form at the bottom.
This form can be mailed to camp or brought on the day of camper check-in.
This form must be completed and signed by a parent or legal guardian.

Confidential Form
This form can be filled in by a parent or guardian. It will provide your child's counselors with some information about your child's needs and expectations.
It can be mailed to the camp or brought on the day of camper check in.
Camper Confidential Form
Check Out Form and Medicine Slips
The camper check out form is required by state regulation to help us determine who is authorize to take your child home.
This document also contains four
medicine slips. These slips must accompany all prescription medicines that your child may be taking during camp. They provide the Nurse with information about the
prescription.

If you are unable to print these forms please use this form to request that they will be sent to you by mail. The forms will be mailed to the address listed for that camper.
