You keep this information for your records.
- Application Form - You
must send this to camp.
- General Reference Form
- This form must be completed by a professor, teacher,
employer, who can describe your personal maturity, and
work ethic.
- Christian Reference Form
- This form must be completed by a pastor or other qualified
person who can describe your Christian experience.
