What to Bring
A complete packing list is included in your Staff Handbook (This handbook was mailed with your agreement).

Medical Forms
All staff must provide, upon arrival at camp or at the Staff Training (whichever comes first), a completed Medical Form. If the staff member has had a physical within the past 36 months, a copy of said physical may be provided. However, all staff must complete Sections A, B, & D and signed by a parent if under 18 years of age. Download a copy of the Staff Medical Form.

W-4
All salaried staff must submit their W-4 by their start date. Download a W-F form now.

To help our Staff Training run in a more efficient way, we are asking that you fill out the form below and return with payment to the Ranch by June 1^st.

NOTE: The Staff Training Week is not a paid week of work.

Staff Training

	I will be attending Staff Training
	I cannot attend Staff Training (only with approval from Olen Johnsen).

	I want to order a staff picture ($5.00), which will be taken during Staff Training.
	$11.00 for the staff shirt (All staff are required to buy a current staff shirt)

	I will be attending the Dedication Service, Sunday June 16th at 10:45 A.M. at Midwest Bible Church.

Summer Staff Arrival
Please indicate the day and time of your arrival. This time should be consistent with the start date listed on your staff agreement, or with prior arrangements with Olen Johnsen. Please fill in this section of the form, in any case, for camp staff to use as verification with their existing records.

Phantom Ranch will gladly pick-up staff and team members at Mitchell Field, the Amtrak Station, or one of the bus stations in Milwaukee free of charge. If using Chicago transportation facilities, there will be a charge of $50.00. Please fill in the following if you desire transportation from one of these areas.
